The aligners themselves are only part of the picture. The substantive difference is supervision. With treatment through a dental office, a qualified clinician examines your mouth in person, takes the necessary diagnostic records, plans the tooth movements, and monitors what actually happens as treatment progresses. Direct-to-consumer systems generally rely on impressions or photographs you take yourself, with little or no in-person examination at any stage.

Because moving teeth is only safe when the foundation is sound. Active gum disease, untreated decay, a failing root canal, or bone loss all need to be identified and addressed first. Applying orthodontic force to teeth with compromised support can worsen the underlying problem and in some cases lead to teeth being lost. Photographs cannot reveal any of this, which is why an examination and appropriate imaging come first.

Teeth can be moved into a bite that looks straighter from the front but no longer meets correctly, which can lead to uneven wear, jaw discomfort, and difficulty chewing. Roots can be moved outside the supporting bone. Existing problems can be masked while progressing underneath. The difficulty with unsupervised treatment is not only that these things can happen but that nobody is checking whether they are happening.

The advertised price is usually lower, and that is a genuine difference. What the comparison often leaves out is the cost of correcting a result that did not go to plan, which can substantially exceed what supervised treatment would have cost at the outset. It is worth weighing the initial saving against that risk rather than against the sticker price alone.

Remote systems vary considerably in what support they offer, and resolving a problem at a distance is inherently harder than having someone look in your mouth.
